Registered Nurse (RN) / Care Manager (CM)

Position Summary:

Our Care Management team is seeking either a Registered Nurse (RN) Care Manager or a Care Manager to provide personalized care and support to caregivers and their loved ones. No night shifts, weekends, or on-call responsibilities are required! This position requires travel throughout Dorchester, Roxbury, Hyde Park, and surrounding areas.

What You Will Do:

• Conduct home visits in assigned territory to assess home safety, gather changes in health status, and provide clinical coaching and support including review of medications
• Develop trusting relationships with caregivers to influence change and reduce caregiver burnout
• Perform other duties as assigned.

What You Will Bring:

• Active Registered Nurse (RN) license in the state of employment or a bachelor's degree (for Care Manager candidates)
• Minimum two years of recent experience in direct care of elders or adults with disabilities
• Ability to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required
